On July 1, 2010, we will add a monthly quantity limit for diabetic testing supplies to the Basic and Standard Quantity Management programs. We will limit coverage to 200 test strips, 200 lancets and 200 needles each month. If your pharmacy benefits do not cover diabetic supplies, then this change will not affect you.
On July 1 we will also add a daily quantity limit for the brand-name drug Exalgo to the Basic, Standard and Core Quantity Management programs. We will limit coverage to two tablets per day, with medical necessity exceptions available through the Prior Authorization department.
If you need more than two Exalgo tablets per day, ask your doctor to call the Caremark Prior Authorizations department at 800-294-5979. Or, your doctor can fax a request to 888-836-0730. On behalf of your health plan, Caremark administers the Quantity Management program. Caremark is an independent company that manages pharmacy benefits.
