Quantity Management Program Changes

On August 20, 2010, we will add a monthly quantity limit for the brand-name drugs Dulera and Zuplenz to the Basic, Standard and Core Quantity Management programs.

• We will limit Dulera coverage to one inhaler per month.
• We will limit Zuplenz coverage to six or nine oral patches per month, depending on the prescription strength, with medical necessity exceptions available through the Prior Authorization department. If you need more Zuplenz than your coverage allows, ask your doctor to call the Caremark Prior Authorizations department at 800-294-5979. Or, your doctor can fax a request to 888-836-0730. On behalf of your health plan, Caremark administers the Quantity Management program. Caremark is an independent company that manages pharmacy benefits.
