2011 Drug List Changes

We will change three of our drug lists on January 1, 2011:

• Preferred Drug List – We will add four drugs and remove nine drugs. 
• Prior Authorization Drug List – We will add 39 drugs to our Standard list. The Expanded Prior Authorization Drug List will no longer apply to any members.
• Step Therapy Drug List – We will add 29 drugs and remove one drug.
