Prescription Drug Update

Preferred Drug List Update

Based on the recommendations of doctors and pharmacists, we are making changes to our Preferred Drug List (PDL), effective July 1, 2012. These drugs are moving from the PDL to the Non-Preferred Drug List:
  • Lipitor
  • Lexapro, Geodon, Zyperxa
  • Caduet, Cardizem CD
  • LoSeasonique
  • Boniva (oral)
  • Combivir, Epivir (tab)
  • Malarone
  • Avapro, Avalide
We are adding these drugs to the PDL:
  • Alvesco
  • Janumet XR
  • Durezol
  • Exalgo

Prior Authorization Changes

As of April 15, the drug Subsys (fentanyl) requires prior authorization. Members can have their doctors call the Caremark Prior Authorization department at 800-294-5979. On behalf of BlueCross BlueShield of South Carolina, Caremark assists in the administration of the program. Caremark is an independent company that provides pharmacy benefits management.

Step Therapy Changes

Starting July 1, we are adding these Second Choice drugs to the Standard Step Therapy Program:

  • Clozaril, Geodon, Risperdal, Zyprexa
  • Alsuma, Imitrex
  • Qnasi

Quantity Management Changes

We added these drugs to the quantity limits list. Our records indicate that no members are affected by this change. Plans with this program will limit coverage to daily or monthly supplies of these drugs:

  • Bydureon
  • Nucynta ER
  • Oxecta
  • Cymbalta
  • Subsys
  • Intermezzo
  • Hemophilia drugs

Communications

We will notify groups immediately and have mailed letters to affected members. View the documents.
