New Individual Packets

Beginning August 1, 2010, our member packets will include this information:

  • Acceptance letter
  • ID card
  • Original application
  • Riders (if applicable)

Members can request a copy of their schedules, policies or amendments by calling customer service at 800-868-2500, ext. 46401. We will mail printed copies of these materials within 5–7 days. We will e-mail copies immediately.
