Quantity Management Program Changes

Effective immediately, we will add a quantity limit for the brand-name drug Onsolis to our Quantity Management programs.

We will limit Onsolis coverage to 120 units per month, with medical necessity exceptions available through the Prior Authorization department. If you need more Onsolis than your coverage allows, ask your doctor to call the Caremark Prior Authorizations department at 800-294-5979. Or, your doctor can fax a request to 888-836-0730. On behalf of your health plan, Caremark administers the Quantity Management programs. Caremark is an independent company that manages pharmacy benefits.
