We realize our members' healthcare benefits are important to them. We strive to offer members affordable coverage with excellent benefits and customer service. Together, we can work to control the increasing cost of healthcare and coverage by keeping accurate records for our members and their families.
Members may receive forms from us in the mail asking them for information. When they return the forms, members help us pay their claims correctly and quickly. Delayed or missing information may affect the payment of a member's claims.
We may request these types of information:
- If they have additional group health insurance (Coordination of Benefits).
- If they have Medicare (Medicare Coordination).
- If they were injured at work (Workers' Compensation).
- If another party may be liable for their injuries (Subrogation).
- If they have a dependent who is over the age of 18 (Student Verification).
- If they have a certificate of creditable coverage (Pre-Existing Condition Exclusion).
We value the privacy of our members' personal health information. We design the methods for sending us information to protect their privacy. These methods comply with the Health Insurance Portability and Accountability Act of 1996 (HIPAA). If a member requests that you submit information on his or her behalf through our Web site, you can rest assured that My Insurance ManagerSM† is secure. My Insurance Manager gives members and benefits coordinators safe, fast access to benefits information.

Need to update Coordination of Benefits (COB) information on behalf of a member? Fill out and submit the COB questionnaire online. First, log in to My Insurance Manager and choose Other Health Insurance. Then, click the Continue to Questionnaire button and follow the step-by-step instructions.
