The State Health Plan fee schedule for 2008 is available for download in an Excel file format. The new schedule will take effect on Jan. 1, 2008. In order to download the file, you must first login to My Insurance ManagerSM† . If you do not yet have a profile, select "Create a New Profile" to get started.
Once you have logged in, select the link to download the fee schedule under the State Health Plan Fee Schedules heading. You will first see the physician network agreement information, then the terms of use. Once you've accepted the terms, you can download the file to your computer.
The file includes the type of service, procedure codes, modifiers, nomenclature, non-office allowances and office allowances. It also contains several worksheets corresponding to the following information:

- 2007 State Health Plan Fees
- 2007 Injection & Drug Fees
- 2007-2008 Anesthesia Fees
