Want to avoid claim denials due to "other health information requested"?  Read further ...

At the beginning of each new year, we require our members to update their Other Health Insurance (OHI) information. Until now, members always had to fax or mail this form to us. Now, members can update OHI online!

You can make it easy for them by giving them computer access right in your office. Ask them to log into My Insurance Manager and update their information. (Members can also access My Insurance Manager from the SouthCarolinaBlues.com home page.) When the member signs on to My Insurance Manager, there is a link to the Other Health Insurance information, including Medicare and the Other Dental Coverage Questionnaire.

If you'd still prefer a hard copy, you can get it here: Other Health Insurance Questionnaire. You may save this form on your computer and e-mail it to members as needed. A Spanish version of this form is also available.
