Medicare Advantage PFFS Terms & Conditions Finder
Enter the first three letters of the member's identification number on the BlueCross BlueShield Medicare Advantage PFFS card. Then, click "Go" to view the BlueCross BlueShield Medicare Advantage PFFS Plan's terms and conditions. If the first three letters of the member’s identification number are "XDV," please choose the appropriate plan name from the menu before clicking "Go." Otherwise, it is not necessary to select a plan name.
