Cholesterol Management

According to the American Heart Association, more than 105 million American adults have high cholesterol. High cholesterol can lead to serious health issues like coronary heart disease and strokes. We offer a program to help your employees lower their total cholesterol levels.

Cholesterol is a waxy, fat-like substance made in the liver and found in certain foods, such as dairy products (whole milk), eggs and meat. The body needs a certain amount of cholesterol. However, elevated cholesterol levels can cause issues. High cholesterol, also called hyperlipidemia, has been linked to many serious diseases including:

  • Coronary heart disease
  • Stroke
  • Peripheral vascular disease
  • Diabetes
  • High blood pressure

While employees with high cholesterol may not impact your bottom line today, they will in the future. Addressing high cholesterol problems early could not only impact your bottom line with higher productivity and lower medical costs, but it could literally save lives.

Solution

The Cholesterol Management program offers participants tools and support to make the necessary lifestyle changes to lower their cholesterol levels. Lowering cholesterol levels is one important way to help employees avoid serious health issues that cause them to be out of work for an extended period of time. When your employees are healthy, they are more productive and are absent from work less.

We identify participants through claims information and automatically enroll them in the program. A letter informing them about the program along with educational materials and a request to complete a simple survey is sent to their home.

Cholesterol Management participants may receive*:

  • A welcome letter introducing the program and instructions to complete the online or telephonic survey
  • Collateral materials to complement the program such as a cholesterol management patient guidebook to help participants focus on ways to lower their cholesterol levels, as well as educational tools regarding healthy lifestyle choices
  • Unlimited access to a personal health coach who can provide support to enrolled members
  • Members identified as high risk will receive counseling calls from their personal health coach to discuss their cholesterol levels, medication, the importance of medication compliance and lifestyle changes that can help lower those numbers

Integration and Tools

Cholesterol Management is a stand-alone product focused only on ways to lower cholesterol levels. If you offer other My Health EssentialsSM programs such as Weight Management, Cholesterol Management can be partnered with them. Cholesterol Management targets members with no other chronic health risks.

Members who have a chronic condition will have access to Cholesterol Management information if they are enrolled in one of our disease management programs.

Employers receive aggregate data of the participants’ results to help you understand the health status of your employees.
