Quantity Management Program Changes

On October 1 we will add a quantity limit for the brand-name drug Qualaquin to the Basic, Standard and Core Quantity Management programs.

We will limit Qualaquin coverage to 42 capsules and one seven-day supply per year, with medical necessity exceptions available through the Prior Authorization department. If you need more Qualaquin than your coverage allows, ask your doctor to call the Caremark Prior Authorizations department at 800-294-5979. Or, your doctor can fax a request to 888-836-0730. On behalf of your health plan, Caremark administers the Quantity Management program. Caremark is an independent company that manages pharmacy benefits.
