Prior Authorization and Step Therapy Program Changes

We are adding the new prescription drug Veltin to our Prior Authorization and Step Therapy programs for anyone 30 and older.

• Members who are 30 or older who are subject to the Prior Authorization program will need to get prior approval before they have coverage for Veltin prescriptions.
• Members who are 30 or older who are subject to the Prior Authorization and the Step Therapy programs will need to get prior approval only if they have never received a prescription benzoyl peroxide product under their benefit plans.

Members who need prior approval for Veltin prescriptions should ask their doctors to call the Caremark Prior Authorizations department at 800-294-5979. Or, doctors can fax requests to 888-836-0730. On behalf of BlueCross BlueShield of South Carolina, Caremark administers the Prior Authorization and Step Therapy programs. Caremark is an independent company that manages pharmacy benefits.
