Quantity Management Program Change

Effective immediately, we will add a quantity limit for the brand-name drug Kapvay to the Quantity Management program. We will limit Kapvay coverage to 120 tablets per month. If you need more Kapvay than your coverage allows, talk to your doctor about your options.
