The State Health Plan fee schedule for 2011 is available to download in an Excel file format. The new schedule took effect January 1, 2011. In order to download the file, you must first log into My Insurance ManagerSM. If you have never used My Insurance Manager before, visit our Provider Tools section to learn more. Once you have logged in, scroll down the page and find the "State Health Plan Fee Schedules" heading. Follow the link. You will first see the physician network agreement information, then the terms of use. Once you've accepted the terms, you can download the file to your computer. The file includes the type of service, procedure codes, modifiers, nomenclature, non-office allowances and office allowances. It also contains several worksheets:
