These CPT codes require additional documentation to determine if benefits are payable. For appropriate medical review, we will need both the laboratory results and the ordering physician’s office notes for the 30 days surrounding the order of the laboratory tests.

To submit medical records, attach the documents to a Medical Review Request Form. This form is in the Provider section of www.SouthCarolinaBlues.com under Forms/Other Forms. Submit the records to the appropriate line of business based on the member’s policy. Please refer to the contact information on the form to make sure the records get routed to the appropriate line of business.
