Credentialing Medicare Advantage Providers

Here are instructions for joining and updating your participation in the BlueCross BlueShield of South Carolina Medicare Advantage PPO network.

If your initial uniform credentialing application is less than 180 days old, you will need to provide this information:

1. A current dated and signed attestation verifying the correctness and completeness of the application that we have on file
2. An updated Health and History form
3. Updated work history
4. Updated proof of current malpractice coverage 

If we have not received a uniform credentialing application within the past 180 days, you must submit the uniform credentialing application to join our Medicare Advantage PPO network.
