• Agents can request a copy of 1095-B forms by: 
  • Contacting your Dedicated Account Advocate (DAA).
  • Emailing Group.Membership@bcbssc.com.

• Members access their 1095-B forms by logging in to My Health Toolkit.
• Agents can request a copy of 1095-B forms by: 
  • Contacting your DAA.
  • Emailing Group.Membership@bcbssc.com.
• Groups must complete and submit forms 1094-B and 1095-B for each employee.

Individual Coverage Health Reimbursement Arrangements (ICHRA)

• Members access their 1095-B forms by logging in to My Health Toolkit.
• Members enrolled on the Federally Facilitated Marketplace (FFM) will receive a 1095-A form via email from the FFM.
• Groups must complete and submit forms 1094-B and 1095-B for each employee, including those who are enrolled in ICHRA and those who waived ICHRA.
