Level Funded Plans

Individual Coverage Health Reimbursement Arrangements (ICHRA)

• Members access their 1095-B forms by logging in to My Health Toolkit.
• Members enrolled on the Federally Facilitated Marketplace (FFM) will receive a 1095-A form via email from the FFM.
• Small Groups (2-50 lives) must complete and submit forms 1094-B and 1095-B for employees who elected to participate in the ICHRA arrangement.
• Large Groups (50+ lives) must complete and submit forms 1094-C and 1095-C for all employees (including those who did not elect to participate in the ICHRA arrangement).
