What’s a PCMH?

A PCMH is a doctor’s office that’s met special criteria to be considered a patient’s medical “home.”

With this model of care, you have a single trusted doctor, who oversees your care. You also get the benefit of working with a health care team dedicated to help you improve your health. You’ll work together to set goals and develop a plan for meeting them. 

The goal is to build a trusting relationship between the patient and care providers to enhance quality of care — and value.

Is a medical home right for you?

BlueCross BlueShield of South Carolina has PCMH practices in our network across the state. Anyone can use a PCMH for their primary care. Some health plans even offer discounts when you receive care through a PCMH.
