You can easily contact our Provider Education department to connect with a provider advocate or to request training. The easiest way is through one of these online forms:
- Provider Education Contact Form – Use this form to connect with a provider advocate for questions not related to specific claims or members.
- Provider Advocate Training Request Form – Use this form to request provider training. You can select from multiple topics and training formats.
- Face-to-Face Visit Request Form – Use this form to request a face-to-face visit from your Provider Education Representative.

We’ll assign your online or phone inquiry to the provider advocate who can most efficiently handle your request. The person who responds may not be the advocate assigned to your geographic area.
