As of Jan. 1, 2026, Lupron Depot transitioned from coverage under the pharmacy benefit for State Health Plan members. Claims for this medication must now be billed through the member’s medical benefit.

Prior authorization is still required before administering this medication. Authorization requests must be submitted through MBMNow, which is accessible via My Insurance Manager℠.
