If you’ve submitted a credentialing application or other provider file update request (Request to Add or Terminate Practitioner Affiliation, Change of Address Form, NPI Provider Notification Form, etc.),  and are in need of a status update, please contact your BlueCross BlueShield of South Carolina and BlueChoice^® HealthPlan Provider Relations and Education Advocate. Provider Education Advocates are designated to providers based on the county in which they are located.

Your Education Advocate will notify you of the status of your file and if additional information is needed to complete it. If additional information is needed, please provide that as quickly as possible so your file can be processed. The Provider Enrollment process cannot be completed until all requested information is received and reviewed.
